Good Shepherd Centres Hamilton, a ministry of the Brothers of the Good Shepherd, strives to enhance the quality of life for all who seek assistance and support in our community. Good Shepherd’s Youth Services – Notre Dame provides emergency accommodation, outreach and support services to homeless and at-risk youth. An opening currently exists in this program for: Early Diversion Case Manager – Full Time The Community Youth Housing Project is a collaborative initiative that emerged from the Street Youth Planning Collaborative. The project is lead by the Catholic Children’s Aid Society of Hamilton in partnership with Good Shepherd – Youth Services, Wesley Urban Ministries, Living Rock Ministries and the Social Planning and Research Council of Hamilton. The DCM role is a position that is based at the Good Shepherd – Notre Dame House and strongly involves in working in the community with youth and service providers. The purpose of the DCM is to prevent homelessness through supporting youth who are at imminent risk of or who are new to their experience of homelessness/stay in shelter. The DCM will connect with youth ages 16 – 21 years at Notre Dame Youth Shelter and in the community to offer services such as shelter diversion, family reunification and assistance to find and obtain safe housing. The DCM will be working with youth to identify areas of support to work towards, such as housing, education, employment, family etc.) and to co create plans of care that enable each youth to attain their goals. They will be actively working with youth to pursue their goals, which may require one on one support with the youth, group work, or connecting with community supports. .The role requires that services are delivered from a positive youth development orientation and from an anti-oppressive/anti-racist framework. Qualifications for the successful DCM candidate includes: Completed post-secondary education in child and youth work, social services, or related fields Applicants with “Lived Experience” of youth homelessness are strongly encouraged to apply 2 years case management experience working with youth 2 years working with marginalized youth, such as youth at risk of or who have experienced homelessness Knowledge of community services for youth in Hamilton Experienced at building relationships with community partners Ability to work collaboratively within a team and also take initiative Written and verbal communication skills Experience with documentation, data collection and reporting Ability to work flexible hours which may include evenings and weekends Additionally, the candidate must hold a valid driver’s license and CPR/First Aid certification. Hours will be 35 hours a week Tuesday to Saturday.
